Each case record includes case/admission number; admission date; patient's name, age, county of residence, marital status, birthplace, and physical description; insanity duration and cause; family background and medical history; prior institutionalization record; support (e.g., private; county); and dated notes on treatment, discharge, or death. Patient's occupation and religion occasionally are given.
